In-house Community Education is provided by our team of Nurse Practitioners who have years of post-acute care experience. This service is offered to our partnered facilities and aims to improve care quality from within the facility and to support your nurses making quality decisions that positively impact care of your residents.

Know Before You Call
Know-Before-You-Call (KBYC) was the first video that laid the groundwork for our comprehensive educational programs. It serves as a reminder to the nursing staff have the necessary information ready before calling the provider. Because education is essential to what we do at Premier Geriatric Solutions, we dig deep into the topics we cover with our facility staff to continuously improve the care we give as a team. We believe that quality communication can reduce medication errors, improve readmission rates, and overall improve patient outcomes.

Infection & Prevention Control
At Premier Geriatric Solutions, we know the value of learning infection prevention and control. Having standardized infection control guidelines prevents hospitalization and improves overall outcomes for patients. Therefore, we educated on this topic in our PACNEP videos and developed four infection prevention and control modules specific to CNA certification. See the website below to learn more.
PGS/NAHCA Water Tool
Premier Geriatric Solutions partnered with the NAHCA (National Association of Health Care Assistants) and developed a communication tool called the WATER tool that functions to improve the communication from CNAs to the clinicians.
As explained in the downloadable PDF below, the WATER tool empowers CNAs to communicate issues noted on their shifts. Given that CNAs are often the first line of care for the patients/residents we serve, it is imperative we focus on this aspect of communication to prevent resident decline and hospitalization.
